If you have subscribed to the www.myalphakit.com client portal service and choose to unsubscribe, contact GeneAidyx at 866-522-0692. If you have subscribed and your patients do not give their consent for you to receive their laboratory results through the www.myalphakit.com client portal, please contact GeneAidyx and their information will be removed.
If your healthcare provider is subscribed to receive your testing results through the www.myalphakit.com client portal, and you do not give your consent for this, please contact your physician so that he/she may contact GeneAidyx.
Information Regarding www.myalphakit.com
GeneAidyx collects information under the direction of its Client the healthcare provider, and has no direct relationship with the individuals whose personal data it processes. If you are a patient of one of our healthcare providers and would no longer like to be contacted by this Client of our service, please contact the healthcare provider that you interact with directly. We may transfer personal information to companies that help us provide our service. Transfers to subsequent third parties are covered by the service agreements with these healthcare providers.
GeneAidyx has no direct relationship with the individuals whose personal data it processes. An individual who seeks access, or who seeks to correct, amend, or delete inaccurate data should direct their query to GeneAidyx’s Client (the healthcare provider). If requested to remove data we will respond within a reasonable timeframe.
We will retain personal data we process on behalf of our Clients for as long as needed to provide services to our Client. GeneAidyx will retain this personal information as necessary to comply with our legal obligations, resolve disputes, HIPAA regulations, and enforce our agreements.

If we make any material changes we will notify our Client by email or by means of a notice on this Site prior to the change becoming effective. Companies that provide services to help us with our business activities, such as our database management group, may have access to your personal information. These companies are authorized to use your personal information only as necessary to provide these services to us.
Grifols provides the FDA approved drug for this deficiency when it is requested by the physician directly. Grifols also distributes Alphakits to healthcare providers. We only share the healthcare provider’s name and NPI number to Grifols. Once the healthcare providers register through our service portal, it is automatic that the information is shared to Grifols. We do not disclose any testing results from any referring healthcare provider to Grifols.
We may disclose your personal information as required by law, such as to comply with a subpoena, bankruptcy proceedings, or similar legal process when we believe in good faith that disclosure is necessary to protect our rights, protect your safety or the safety of others, investigate fraud, or respond to a government request. If GeneAidyx is involved in a merger, acquisition, or sale of all or a portion of its assets, you will be notified via email and/or a prominent notice on our website, of any change in ownership, uses of your personal information, and choices you may have regarding your personal information.
